We see patients from 9 am to 4 pm Monday through Friday.

How do I make an appointment?

To make an appointment, please call 423-777-5900. However, walk-ins are also welcome!

Where are you located?

We are located at 5109 Brainerd Road, Chattanooga, TN 37411, between Cook Out and the Fire Station along North Howell Avenue.

Are you offering COVID-19 testing?

Yes, we test for COVID-19 at our office.

Are you offering flu shots?

Yes, flu shots are available at our office.

Can I get my bloodwork done here?

Yes, our office offers in-house lab draws.

What do I do in case of an emergency?

If you are experiencing a life or death situation - CALL 911 or go to the emergency room.

If you have ingested plants, drugs, poison, etc., call POISON CONTROL immediately and follow their directions. 800-222-1222

What forms of insurance do you accept?

We currently accept:

Aetna, Ambetter, Amerigroup, Amerigroup Community Plan, BCBS, Bluecare, CareSource, Cigna, Cigna Healthspring, Medicaid (TN and GA), Medicare, TennCare, UHC Community Plan, and United Healthcare.

What forms of payment do you accept?

We accept cash, personal check, debit card, and credit card.
